Thank you for asking me the question. I think I've learned something new as well.
In the military, "about face" is an order to make a sharp, sudden 180-degree turn when standing in line. So "to do a complete about face" means to change to the exact opposite as you were--in physical stance, opinion, health, whatever--very quickly and suddenly.
